Full Proposal

A Full Proposal should be 5-10 pages of Narrative accompanied by Financial information. The Proposal should address the following topics:

 

NARRATIVE

  • the need the proposed program addresses
  • the program’s Intended Impact*
  • the activities used to achieve the program’s Intended Impact*
  • the program’s Theory of Change*
  • anticipated obstacles or challenges to achieving the program’s Intended Impacts* and the strategies for addressing these obstacles/challenges
  • the evaluation plan for the project, if one exists, describing how progress toward the various Intended Impacts will be measured and reported
  • contact information for the primary contact person
  • the qualifications of staff members integral to the success of the program
  • the amount requested, including the time period the request covers
  • the dates of the project year
  • the organization's fiscal year

FINANCIAL

  • a budget for the program
  • a budget for the organization
  • the most recent year-to-date report that shows actual expenses v. budget for the program
  • the most recent year-to-date report that shows actual expenses v. budget for the organization
  • a list of funds committed or received for the project. The list should include the following information - name, amount, when awarded, time period the grant covers, and any restrictions on the award (e.g., general operating support v. restricted to a specific program or use)
  • a list of funders approached and grant requests pending for the project. The list should include the following information - name, amount, time period request covers, expected decision date, and target of request (e.g., general operating support v. restricted to a specific program or use)
  • a list of funds committed or received for the organization. The list should include the following information - name, amount, when awarded, time period the grant covers, and any restrictions on the award (e.g., general operating support v. restricted to a specific program or use)
  • a list of funders approached and grant requests pending for the organization. The list should include the following information - name, amount, time period request covers, expected decision date, and target of request (e.g., general operating support v. restricted to a specific program or use)

FIRST TIME APPLICANTS

 

If this is the first time you are applying to the Foundation, please also submit:

  • statements of actual income and expenses for the most recent two years for the project
  • statements of actual income and expenses for the most recent two years for the organization
  • balance sheets/statements of financial position for the organization for the most recent two years (include an audited statement for the most recent fiscal year available)
